- Family Coaching and Care Coordination (Planned addition in 2009!): Through Family Coaching and Care Coordination families become functional, children are healthier, safer and properly cared for, students succeed in school, parents find employment, seniors can maintain their health and sustain independence longer. A collaborative approach to service where the unique needs of each customer are the focus of personalized assistance and referrals. Once individual or family goals are agreed upon, Charles House works with each person to address critical problems and attain the help needed. A plan is developed delineating the problems to be overcome, the resources needed to make progress, and milestone accomplishments along the way to achieve desired results. Once goals are attained, a sustainability plan is implemented.
